FINANCIAL RESPONSIBILITY FILINGS
SR-22 applicants are only eligible for the purchase of the minimum limits (20/40/15).
A $10.00 processing fee for all SR22’s will be charged to the agent.
Filings required due to DUI offenses.
· The applicant must not have any DUI offense during the last 12 months and must not have more than one (1) DUI offense during the last 36 months preceding.
AMCO will prepare and forward the SR-22.
1. $100 DPS fee needs to be deposited into AMCO’s account. For agents, amco will sweep $100 DPS fee from agents account.
2. SR-22 information sheet and DPS form DR-1a (signed by customer) needs to be faxed to AMCO toll free fax number in Houston at 1-888-412-2022. Please call to verify fax was received.
3. AMCO’s SR-22 representative will process the SR-22 and mail to DPS the following business day. DPS will mail the original clearance letter and driver’s license (if DPS has it) will be mailed to the Insured’s address listed on the SR-22.
4. We will file SR-26's at the expiration or cancellation.

If
loss of license was due to an accident and policy has to be paid for six months
in advance, DPS requires an
original SR22A form with the notarized policyholder’s signature.

The 515A endorsement must be signed and dated by the Named Insured. The exclusion is required on each person residing with applicant who is 14 years and older, if not rated as a driver of the vehicle(s).
The 551 Endorsement must be signed and dated by Named Insured if the covered auto is required to travel to Mexico within 25 miles of the United States border. Named Insured will be charged additional premium if Mexico Coverage Insurance is purchased thru Amco.
PERSONAL INJURY PROTECTION AND UNINSURED/UNDERINSURED MOTORIST COVERAGE
REJECTIONS
The
Insured will be charged for Personal Injury Protection and
Uninsured/Underinsured Motorist coverage unless the properly signed rejections
are completed in our office. Mailed in Rejections of these coverage(s) cannot be
effective before the postmark date.
RATING DISCOUNTS: Maximum combined discounts - 35%.
1. Multi Car
- (10 %)
Discount
allowed for two or more vehicles on the same policy.
2. Good
Driver - ( 10%) (clear MVR, 6 months prior insurance, and proof)
Driver
must have a clean MVR (no tickets, violations, or accidents for the past three
years) and prior insurance for the past 6 months with no lapse in coverage.
Driver must show proof of insurance to receive discount.
Proof must be a copy of the Declaration Page. ID cards can be used if agent calls previous insurance
carrier to verify coverage and makes a notation of this.
3. Homeowner
- (5%) (own
a home and proof)
Driver
must own a home and be able to show proof.
Proof would be a deed/title to the home, a mortgage payment, a tax
statement, or a homeowner’s insurance policy in the insured’s name. Mobile
homes, townhouses, and condos are included in this discount.
4. Transfer
- (5%) (Prior Coverage)
Discount
will be allowed when the driver has had prior insurance with another carrier
with no more than 30 days lapse in coverage. Driver must show proof of insurance
to receive discount. Proof must be
a copy of the Declaration Page. ID
cards can be used if agent calls previous insurance carrier to verify coverage
and makes a notation of this.
5. Alarm
System/ Lo-Jack - (5%)
(Physical Damage Only)
Given
only when there is a sound remote alarm for the vehicle.
Customer will need to operate the alarm to receive the discount.
Alarm discount is not given when there is a kill switch, a club, a combination
pad on the vehicle, etc. Discount
is also given when Lo-Jack tracking system is installed in the vehicle.
6. Defensive
Driving – (5%)
Given
with proof (defensive driving certificate).
7. Amco Renewal - (1st year 5%,
2nd year +5%, 3rd
year +5%)
Is
given to our Amco customers that are renewing their policy with Amco and have
been claim fee for the past year (12 months).
Maximum discount applies.
Submit a completed Endorsement Request Form for any changes including adding or deleting vehicles, coverage, drivers, loss payees or changing name, address, marital status, etc. to an existing policy.
Any endorsement that creates additional premium must be quoted on a pro rata basis by the producing agent or underwriting and submitted with the request.
Endorsements received without the necessary premium will be
returned with “NO COVERAGE BOUND” for the endorsement.
1 No flat cancellations will be allowed except for check returned for “non-sufficient funds” or “account closed”. Application or renewal shall be null and void and must be resubmitted.
2. Cancellations made by the Company or Named Insured’s request will be calculated on a pro-rata basis.
3. Cancellations done by Named Insured’s must also be signed by the Named Insured.
A renewal offer will be sent direct to the insured offering to renew the policy.
To allow processing time the payment will be due 5 days prior to the expiration date of the policy. There is no grace period allowed. For mail payments, coverage will be effective 12:01a.m., the date after the postmark date. For premium taken at the location, the coverage will be effective at the date and time the premium is taken.
To allow
processing time the payment will be due 10 days prior to the expiration
date of the policy.
If the payment is received and/ or postmarked after the date of
expiration, it will be returned unbound. The
policy must be resubmitted with a new application. For late payments during
a policy period, please refer to reinstatement procedures.
If the policy has expired and the renewal payment is postmarked or received by an agent less than 10 days after the policy expiration, the payment will be accepted, with a lapse in coverage. The effective date will be the date after the postmark or the date received.
After 10 days, an inspection and a “No Loss” statement will be required. If the policy has been expired for a period greater than 90 days, the policy must be written as new business with all applicable fees and attachments.
If the payment is postmarked or received by an agent after 10 days from forwarding of cancellation notice to the insured, the policy will be reinstated, with no lapse in coverage. However, the time and date the payment was made should be noted, and an inspection and a “No Loss” statement will be required. The insured will be required to pay all accumulated premiums. We will not reinstate any policy more than 90 days after the effective date of cancellation.
All policies cancelled due to “non-sufficient funds” or “Account Closed” must be resubmitted as a new application along with all applicable fees.
Any Driver:
1.
Involved in an accident in the past year (12 months).
No more than one accident during the past 3 years per driver is
acceptable.
2.
Without a valid driver’s license.
3.
Without a driver’s license (except those regaining licenses under SR-22
filing). If customer does not have
a driver’s license with them, charge them 2 points.
Then have them sign a letter stating they will bring in their Texas
Drivers License within 72 hours.
4.
Over the age of 69 years unless a medical report from their physician is
provided stating their driving capability.
5.
With a DWI in the past year
(12 months). No more than one DWI
during the past 3 years and customer must be filing for an SR-22.
6.
With more than 10 M.V.R. points.
7.
Who is in the country temporarily (aliens).
8.
Who is applying for a Six Month policy and has only an International
Driver’s License.

Can
be written on a One Month policy.
9.
Under the age of 18 years unless they are put on a policy with an adult.
The adult needs to be the Named Insured on the policy.
10.
With the following violations in the past 12 months:

Driving
under the influence of alcohol, drugs, or narcotics.
·
Open
container, closed container and other liquor violations not listed as a Major
violation.
·
Attempting
to elude the police.
·
Speed
Contest/Racing.
·
Hit and
Run.
·
Reckless
driving-including without due regard, careless, improper, unsafe and negligent
operation of vehicle.
·
Felony
involving use of motor vehicle.
·
Displaying
altered driver’s license.
·
Unlawful
use of driver’s license.
·
Driving
with license suspended or revoked. (Unless with SR-22 filing.)
·
Failure
to yield to emergency vehicle or school bus.
·
Refusal
of an alcohol test (Implied Consent Law).
·
Negligent
operation of a vehicle.
Amco
will accept one more vehicle than there are drivers.
